FIU's Immunization Compliance Package, item by item
Florida International University (FIU) groups its health requirements for clinical students into an Immunization Compliance Package: influenza and TB every year, varicella, MMR and hepatitis B once each by doses or titer, Tdap every 10 years and a one-time physical examination form. The one-time items can be cleared in a single push; the trouble comes later, when the yearly items expire in the middle of a practicum. This guide covers what FIU accepts for each item and how to keep the dates ahead of your rotations.

The immunization items are the part of clearance you control most directly: no vendor search and no waiting on a former employer to answer a verification request. That makes them the right place to start while a site contract is still being worked out.
Doses or titer: choosing your proof
For varicella, MMR and hepatitis B, FIU accepts either a record of the doses or a titer, the blood test that shows immunity. Working nurses often have much of this on file with an employer's occupational health office or a primary care provider, so ask for copies before you book anything new.
A titer is often the quicker route when dose records are scattered across old schools, jobs or states. It needs a lab draw and a result, so allow time for both. If a titer result raises a question about what to do next, take it to the Office of Clinical Education rather than guessing, since FIU decides what completes the package.
Hepatitis B is the one item with a declination option. FIU allows the Hepatitis B Declination Form in place of doses or a titer. Varicella and MMR have no declination form in the handbook's list.
TB testing: two routes and the positive-reactor path
FIU asks for TB screening every year, by either a 2-step PPD skin test or a QuantiFERON-TB Gold Plus blood test. A 2-step PPD means two separate tests with readings, so it takes more than one visit; the blood test needs one draw and a lab result. Pick the route that fits your schedule in the weeks before a practicum.
Students who test positive follow a different path. The handbook asks positive reactors for a chest X-ray every 3 years and a TB Clearance form every year. The college also lists an NWCNHS TB Annual Screening Form among its Clinicals & Fieldwork documents; use the version posted there.
Because TB renews every twelve months, its date drifts through the program. The test you took in August will be due again in August, which may be the first week of a Fall practicum.
Put your TB and flu expiration dates next to your practicum start dates now; the yearly items are the ones that lapse mid-rotation.
Flu and the other yearly dates
Influenza is due every year under FIU's list, and proof of health insurance is also yearly, so a student in the program collects a small set of renewals every twelve months. The handbook does not tie them to a single month, which means each one runs on the date you last completed it.
Check them against the term you are about to enter. For 2026-27, Fall runs August 24 to December 12, Spring runs January 11 to May 1, and Summer Session C from May 10 through July 30. If any item would expire before the last day of the term, renew it in the break before the term starts.
Tdap sits on the slowest clock, every 10 years, but check its date once at admission; a dose from nine years ago will come due during the program. The academic calendar guide lists the session dates in full.
The physical exam and the health forms
The physical examination form is a one-time item in the package. The college also posts an NWCNHS History & Physical Form on its Clinicals & Fieldwork page, and a healthcare provider has to sign the one-time FIU Performance Standards Form.
Since all three involve a clinician, one appointment can usually cover them, along with any titer draws or a TB blood test. Bring the current versions of the forms with you so the provider completes the right pages. Scan each completed page the same day and store the files with your titer and TB reports, so a site's onboarding request can be answered in minutes.

When a site asks for more
FIU's package is the baseline. The handbook anticipates facilities that want more than the package, and any extra screening they ask for is at your expense. Some large systems also run their own onboarding, with deadlines counted in business days before the start; the clearance overview gives one example. A site's extra requirement sits on top of FIU's: you complete both, and FIU's package still has to be current for the whole rotation. Any such extra item can mean another lab draw or provider visit, so it belongs on your calendar as soon as you know about it.

Questions FIU students ask
Does FIU accept a titer instead of vaccine records?
Yes, for varicella, MMR and hepatitis B. FIU's handbook accepts either documented doses or a titer for each of the three. Tdap, influenza and TB have their own requirements. If a titer result leaves you unsure what else is needed, ask the Office of Clinical Education before scheduling more shots.
Can I decline the hepatitis B vaccine for FIU clinicals?
FIU's handbook allows a Hepatitis B Declination Form in place of doses or a titer. It is the only declination form in the Immunization Compliance Package. Individual clinical sites may have their own expectations, so ask a prospective site about its policy before you rely on the declination there.
What happens at FIU if my TB test is positive?
The handbook asks positive reactors for a chest X-ray every 3 years and a TB Clearance form every year. Keep the X-ray report and the yearly form in your clearance account, and check the dates before each practicum semester. The TB Clearance form is the yearly piece to watch.
How often do FIU nursing students need a TB test?
Every year, by 2-step PPD or QuantiFERON-TB Gold Plus. Each test runs on its own twelve-month clock, so a test done just before your first practicum comes due again a year later, often at the start of another clinical term. Renew in the break between semesters when you can.
Where do I upload my immunization records for FIU?
In your clearance account, which the graduate handbook calls Complio and FIU's web pages call FIU Express. Use the link in your admission instructions, and if the two names confuse you, ask the Office of Clinical Education which one applies to your term.
